Filling out your W-9

In accordance with U.S. federal tax regulations, we require all customers to submit a completed W-9 form before we can process redemptions that are a cumulative total of $600. This threshold changes to $2000 for year 2026. 

Our team will reach out to players who cross this threshold and you will be sent the following link to complete this:


On the first page you will have to input information into the following 4 areas as seen below:

1. Your Company / Business Name - This needs to filled out with your First and Last Name
2. Your Full Name - This also needs to be your First and Last Name
3. Email Address - The email address associated with your account
4. Phone Number - The phone number you use when logging into the platform



The next page shown to you can be left as is so you can proceed to the next page.




The following page will have you provide the following information:
1. Legal Entity Name - This should be your First and Last Name
2. Entity Type - You need to input Individual / Sole Proprietor here since you're filling this out on the behalf of yourself, we do not allow the use of any LLCs or businesses for the purposes of this form.
3. Tax Identification Number - SSN - This needs to be your SSN with no dashes or spaces, the example has these included for illustrative purposes only.

Remaining information on this page needs to be left blank.



On the next page you will asked for the following address information:

1. Address - This is where you will input your Street Number / Street Name / Unit Number / Apartment Number if applicable. This information all needs to go on one line so we recommend not using an auto-fill tool to do this.
2. City - The city you reside in
3. State - The state you reside in, if your state isn't available in the drop down then you should be able to type this in manually
4. Zip Code - The zip code where you reside


If you have moved and the information is different from the information that was on your ID at the time of registration we will ask you to provide one of the following documents so we can update your information on our end.
- A recent Utility Bill
- A recent Bank Statement or other similar document clearly detailing your residential address (just the page showing your address and account number, please redact transaction history and balance if you like)

The next page will ask you the following information but you nothing on this page needs to be filled out, you can go to the next page from here.


The last page will show you a recap of everything you've already filled out and you will also be required to do the following:
1. Signature - Sign your name here
2. Submit - You're all done!


You'll then be able to download a PDF version of this form for your own records if you choose and this is how the final form looks when completed.


There is no need to do anything else. Your submission will be validated and your redemption request processed as soon as possible during standard business hours. Overnight submissions will be processed first thing the following morning.
